7 Day Free Trial
You have 7 calendar days from your date of sign up to try the website with unlimited submissions at no charge. On the 8th day your account will convert to the plan of your choosing.
You will be billed at the end of your billing cycle. Your billing cycle begins after your 7 day free trial period ends. You will be billed based on the usage on your account, the number of submissions you have used. If you are getting close to the maximum number of submissions in your plan you will be notified by email about a pending upgrade to your plan. We will not cut you off. Your plan will be automatically upgraded to the next tier to ensure your employees and visitors are always able to fill in your covid screening form.
If you would like to stop submissions, simply go into your account and disable your sites. You can edit each site and set it as active or disabled. When a site is disabled, no submissions will be accepted for that site. In order to stop all submissions to your account, simply disable all of your sites.
You may cancel your account at any time. Billing is done at the end of the month for the usage that you have used for that month. When you cancel your account, your account will be marked for cancellation as of your next billing date. You will still be billed for the appropriate plan for the current month and your account will remain active until your next billing date. On that billing date, your account will be charged for the appropriate plan, deactivated, and no further billing will occur beyond that point. Your account will be cancelled.
We do not provide refunds, instead please cancel your account to end your billing. For extenuating and special circumstances, you may request a refund by contacting us from the contact us page and explaining your situation. If your return is approved, we will initiate a refund to your credit card (or original method of payment).
You will receive the credit within a certain amount of days, depending on your card issuer's policies.
